Health Screenings~ Vision & Hearing

Vision and hearing screenings will take place in December. Re-screen date will also be in December 2018.

VISION SCREENING

Vision screening will be done, as mandated, for the following students: all preschool, Kindergarten, Grade 2 and Grade 8 students, Resource students, teacher referrals and all transfer students. Vision screening is not a substitute for a complete eye and vision evaluation by an eye doctor. Your child is not required to undergo this vision screening if an optometrist or ophthalmologist has completed and signed a report form indicating an eye examination has been administered within the previous 12 months and that evaluation form is on file at school. If a vision examination report is not on file at the school for your child; your child in the mandated age/grade/group will be screened.

HEARING SCREENING

Hearing screening will be done for the following students: all preschool, Kindergarten, Grade 1, Grade 2, Grade 3, Resource students, teacher referrals and transfer students. The student will be screened unless there is a completed and signed report form indicating that the child has had an ear exam by a physician and an Audiological evaluation by an audiologist within the previous 12 months. In cases of hearing loss, an Audiological evaluation completed by an audiologist within the previous 12 months may be accepted instead of threshold monitoring services.
